Cambridge University Lung Transplant study questionnaire

Cambridge University are undertaking a study as part of developing an information tool for people being assessed for transplant. Initially the study is for Lung transplants, with Heart transplants being a later questionnaire.
Leila writes: Hello ! I’m part of a team working to create a Risk Communication Tool for Transplants. I presented the project at the May Cardiothoracic Patients Group Meeting (CTPG).
There is a Participant Information Sheet (PIS), along with a Consent Form; links below. The PIS is very long but it explains in detail what we’re doing and why we’re doing it. If you think you might be happy to help us and take part, please do have a good read of the PIS, I will go over it with you again before we do any interviews.
We are looking for people to talk to us about their experiences and thoughts about a risk communication tool. Interviews can take place over SKYPE or sometimes on the phone, or we can arrange to come to speak to you at your home or somewhere near you, or at your hospital at a routine appointment. Whichever is easiest for you.
